Dear Parent / Guardian:

Your son/daughter has expressed a desire to participate in the Interscholastic Sports Program. Because of the injury risk factor involved in Interscholastic Sports it will be necessary for you, as a parent, to fulfill the following requirements prior to your son/daughter's participation in the program. 

  1. An annual physical examination. It is recommended that this examination be done by the student's own physician who will be knowledgeable about the student's previous health history and will be better able to assess his/her current physical status.
  2. Hospitalization/medical insurance coverage. 
  3. Completion of the Consent Card by the physician and parent.
  4. Completion of the Athletic Emergency Card by the parent. 

The completed Consent Card and Athletic Emergency Card should be returned to the Health Office. If you have any questions regarding the above requirements, please contact the School Nurse at your earliest convenience.
